SDS Rate Study Submission
The SDS Family Group has put together the following guidance to help our stakeholders submit their initial public statements to DMH’s rate study of the SDS program. The deadline for these submissions has been extended to February 16 due to system issues.
The template that DMH has developed is at (https://dmh.mo.gov/media/file/sds-stakeholder-feedback-template). Please complete it, taking note of the following items, and submit it to [email protected].
We encourage EVERYONE to submit their thoughts on each of the items but also incorporate the following pieces of information. This form can be completed AS MANY TIMES as necessary from anyone who has knowledge about the SDS program, is using the program, is affected by the program, or even just knows an individual receiving these services.
